  • MEDICAL TREATMENT RELEASE FORM - This also needs to be filled out and signed every time you wish to go on a Youth Outing.

  • Aldersgate Youth Activity Permission Slip - this form needs to be filled out and turned in each time the Youth Group goes away on an outing. If you're not sure if you need one, bring one!
